I've created a spreadsheet that helped us to get CGMS coverage, along with instructions on how to fill in the form. See the links below.

In order to edit the spreadsheet, you will have to save a copy to your own computer, as it will not allow you to modify the original version:

I've had several people ask about this, so I decided it was just time for me to try and post it online. I'm not real technically astute, so this is my first attempt at publishing documents. As always, drop me an email if you need help with it.
I am still doing my best to try and make this thing so that you can save it to your computer and edit it to serve your own needs. Thanks JR for the tips, but I'm still not sure I am doing it right.

great post. I will add this to my documents for challenging my denial of coverage for the CGMS
ReplyDeleteThanks
It's not really possible for people to get their own copy of that great spreadsheet using the link you posted... You should consider posting the "View only" link rather than the "Published" link...
ReplyDeleteIn the SHARE tab, there is an option for "Anyone can view this document at:" - which will give you a link/URL if you check that box. If people click that link (once you post it), they will enter a session WITH you on that spreadsheet, but will not be able to edit it... but they will be able to do a "FILE" / "COPY SPREADSHEET" to get their own copy in their Google Docs account...
or - you can append the &newcopy option to the end of that URL and anyone who clicks the link will IMMEDIATELY be taken to their own, editable copy...
Check out the other diabetes related samples I've posted at sharingdocs.blogspot.com - I will likely post your there too if you don't mind...
Not sure if I have done it right yet or not. In the meantime, everyone's welcome to email me and I will just email a copy back to you.
ReplyDeleteNick,
ReplyDeleteJust click on FILE EXPORT XLS and save to local computer. That will create a fully-functional Excel spreadsheet on the users computer.
